First, what is a bunion?

WARNING: This article contains graphic images of surgical procedures, and very valuable information!

Bunions (pictured below) are simply a drifting apart of 2 bones in your foot called metatarsals which then forms a bony prominence at the base of the big toe. This area then becomes painful due to pressure when wearing shoes. The outside of the foot near the pinky toe can also have a bunion by the same mechanism called a "tailor's bunion," or bunionette.

What causes the 2 bones to separate? No one really knows exactly why, but the biggest risk factor is simply genetics.

The 2 bones make an angle called the 1st intermetatarsal angle which is measured (in the example below its about 17 degrees), and its used to gauge the severity of a "bunion".

Bunions typically worsen over time, and the longer you wait the harder it is to correct, and the more likely it is that it can come back after fixing the bunion.

How bunions are treated without surgery.

Since bunions are a structural problem there are few remedies that may help. Typically the only thing that may resolve pain are splints (like shown) or simply wearing wider shoes. Splints are cumbersome, and require larger wider shoes, and wider shoes on their own are simply not an acceptable option for most people.

Surgeons

The normal, & old way to do bunion surgery

Most surgeons will perform this surgery to realign the bones in your foot in the intimidating surgery center, or hospital. It is often an all-day affair involving poking, prodding, waiting, and more waiting. You also must be fasting prior to surgery.

Then, you will be put to sleep either by general anesthesia, or IV sedation. One requiring a tube to be inserted down your throat, and other an IV inserted. Both must be performed in a surgical suite because of the risks involved.

After the procedure you are groggy from the anesthesia, and depending on the procedure you may be in a cast or large orthopedic boot. You are then given your prescription for opioids to control your pain during your recovery.

Oh.. the pain, and waiting!

Bunion surgery is notoriously painful to recover from. Why? Primarily because the foot is a small compact structure, with small compartments. The foot swells, and this swelling pushes on these compartments creating even more pain. Also, there are many nerves very close together in the foot. These often are damaged in bunion surgery.

To make matters worse a popular surgery performed is a "Lapidus" pictured here. This fuses a joint in the midfoot requiring extensive time off of your foot which usually means at least 2 months! Some severe bunions require this surgery, but most do not.

After your surgery you learn your surgeon has gifted expensive hardware to you! This hardware remains permanently unless it begins to become bothersome, and then must be removed.
